-Every approved waiver provider must have a Serious Incident Review Committee (SIRC) to provide oversight and review of the investigation process and outcomes of incidents involving clients we serve. There are two types of incidents SRI, Serious Recordable Incident and Internals, a Recordable Incident.

-The SIRC is responsible for reviewing the investigation of every SRI and Recordable Incident to:

·        Evaluate whether it was thorough and objective, and if the conclusions and recommendations comply with standards and guidelines.

·        Determine if it was appropriately reported, investigated, and documented.

·        Ensure Agency policies and procedures are being followed and/or adjusted based on incidents.

·        Ascertain that corrective, preventive, and/or disciplinary action has been taken in accordance with committee’s recommendations.

·        Identify SRI trends and recommend appropriate corrective and preventative policies and procedures to the agency administration.

·        Review Annual Satisfaction Survey.

SRI: These incidents involve more serious situations including but not limited to hospitalization, possible criminal acts, neglect, and abuse.

Internal: These incidents involve falls, injury, behavior monitoring, and seizure monitoring.

-As needed (at least quarterly, 4 times per year. Each meeting lasts between 1 to 2 hours)

-Meetings are held via zoom (in person as needed).

-Commit to attending and participating in scheduled QA meetings

-Actively listen to incident reviews, ask questions, make suggestions, share your expertise and experience.
